Clinical Team Leader

 Role Summary:
 
Manage the activities of the team, partnership with, and support of the Manager, close involvement in efficient and effective management of the ward and nursing practice. Teach and demonstrate procedures to and supervise registered and unregistered nursing staff, using available evidence to inform and develop practice.
 
Promote training initiatives at ward level; take a key role in the induction of all new staff, acting as a role model to team members.
 
Coordinate the process of each patient’s individual care programme through effective delivery of the CPA.
 
To participate in Night shift team leader rotation, to cover annual leave and sickness.
 
Key Responsibilities
 
Patient Care
 
Ensure nursing practice is in accordance with the NMC Code of Professional Conduct and Mental Health Legislation.
In partnership with, and in support of the Manager, manage the nursing team to achieve standards as laid out within the organisational policies and procedures.
Ensure that the patients are involved as fully as possible in decisions affecting them and the running of the units, and to maximise their independence.  
Participate as an active multi-disciplinary team member, making an appropriate contribution to team processes.
Maintain a professional knowledge base using current research and evidence based approaches to care, acting as a resource to others and contribute to the development of nursing standards and audit within a quality framework.
Support the Manager, using such forums as the service development meeting, in securing and maintaining a continuous review process in order to facilitate developments, innovation and improvements in services.
Promote and facilitate nursing practice, which incorporates and respects differences in respect of gender, sexuality, ethnicity, religion, culture and ability.
Promote the application of the care programme approach using clinical leadership to secure effective care planning and evaluation
Meet agreed standards to ensure effective patient care by use of Tools such as HONOS, MANCAS, SNAP and STAR.  You should audit that these are in place and completed regularly.
Act as a Primary/Named Nurse for any designated patients.
Ensure all ward information such as induction booklets are updated and maintained to reflect the specific service
 
Communication
 
Always acting in a dignified positive and professional manner as a representative of the Hospital, on the ward and in all other forums 
Communicate effectively, both in writing and verbally, with other health care professionals (internal & external) disseminating information related to the delivery of care.
Complete factual and consistent records using a problem solving approach, which demonstrate an accurate account of treatment, care planning, delivery and evaluation.
Monitoring of all records to ensure standards are met and maintained.
Refer to other health care professionals and seek guidance where there are limitations in personal knowledge, skills and competence on specific issues relating to the delivery of care.
Ensure that key information is communicated to staff and patients, utilising feedback to generate best practice solutions.
To report any accidents, complaints or untoward incidents to the Clinical Nurse Manager and sign all information related to the above in a timely way.
 
Ward Team
 
Maintaining accountability for management of all ward staffs clinical & management supervision and appraisals (incorporating the RAG document), ensuring completion by allocated staff and the communication of results to relevant parties.
Providing support and guidance to junior nurses in the delivery of their supervision and appraisals to junior staff.
Identifying training needs, supporting staff with their individual and practice development using objective setting and performance measurement tools
Working in the role of Mentor to junior nurses and preceptees
In managing ward activities, match the tasks and care competencies to the capabilities and workloads of team members, either as a member of the ward team or the person in charge of the ward, ensuring the numbers and skill mix of staff are appropriate to the activity and that staff are fully briefed on their role
Contribute to budgetary and human resource management to ensure effective use of resources.
Liaise with Audit Department and accept audit responsibilities.
